PADI Open Water Diver certificate
A student diver can present the PADI Universal referral for Open Water Diver certification to a instructor. It states that the diver took initial training from another diver training organization. This form instructs the instructor to complete the student’s open water dives and sign a statement. The form should then be sent to the original instructor who should certify it.
PADI will require that you complete the course within 12months of your last training. This usually takes two to three days. You must be able and willing to dive with your buddy up to 60 feet/18 metres.

Statement of medical necessity
A Medical Statement for universal Referral Form is required if you are interested in becoming a certified scuba diver. This important document will help you ensure your health before you dive. This document is designed to protect you underwater. Be sure to answer all questions honestly.
First, check that your physician has signed the form. This is a requirement when you first start training as a scuba diver. A new medical release is required if you have a medical condition. A photocopy of your certification card is recommended.
Logbook
A medical statement is required before a diver can open-water dive. This is usually done before certification courses begin. The medical release must be signed by a physician. An updated medical release is necessary for patients with changing medical conditions. A medical release is available through PADI. The PADI Release Of Liability/Assumption Risk/Non Agency Acknowledgement Form-General Training or the EU version is equivalent to the medical release.
The universal referral is different from that used in a diving logbook. It will detail the number of open water dives the student has taken and the knowledge development sections they have completed. It also contains a signed statement from the instructor confirming that the diver has taken the required dives. The student is then required to take the form back to their instructor to be certified as a diver.

The course requirements
The Universal Referral Program is used by most well-known scuba certification organizations. This allows a diver to complete both the theory and pool parts of the open-water course at his or her local dive centre, while also completing the open-water check-out dives at another agency. This can be accomplished with the assistance of a dive shop that participates in a universal referral program, such as SSI/NAUI/NASDSS/YMCA or any other organization.
PADI Open Water Diver training requires students to pass the Knowledge Development, and Open Water Dives sections. A medical release must be signed by the student diver before he or she can dive. The student diver must also provide a photo to accompany his or her certification card. Digital images can be accepted.
