PADI Open Water Diver certification

The PADI Universal referral form for Open Water Diver certification is a document that a student diver presents to a diving instructor. The form states that the diver received initial training at another diving school. The form directs the instructor of diving to complete the student’s open-water dives and sign a declaration. This statement should be sent to the original instructor so that they can certify the diver.

While you can use the Universal referral form to get the certification, PADI requires that you complete the course within twelve months of your last training. It is typically a two- to three-day course. During this time you will need to be able to dive together to a maximum depth 60 feet/18m.

Statement of medical necessity

To become a certified scuba diver, you will need to complete a Medical Statement for Universal Referral. This is an important document that will ensure you're as healthy as possible before you go diving. This document is designed to protect you underwater. Be sure to answer all questions honestly.


First, check that your physician has signed the form. This is required when you start your training as a diver. A medical release is required for any new medical conditions. Last but not least, make sure you have a photocopy to your certification card.

Logbook

Before a diver can dive open water, he or she must complete a medical statement. Generally, this will be done before a certification course begins. A doctor must sign the release. A new medical release is required for any medical condition that changes. PADI provides a medical release. The PADI Release or Assumption of Risk/Nonagency Acknowledgement form - General Training or its EU equivalent, the medical release, is available through PADI.

The universal referral is different from that used in a diving logbook. It will include information about the dives that the student has completed, as well as knowledge development sections. This document also includes a certified statement by the instructor that certifies that the student completed all required dives. The student is then required to take the form back to their instructor to be certified as a diver.

Course completion requirements

The Universal Referral Program includes most of the well-respected scuba certification agencies. This allows a diver to complete both the theory and pool parts of the open-water course at his or her local dive centre, while also completing the open-water check-out dives at another agency. This is done with the cooperation of a dive center with a universal referral program such as SSI, NAUI, NASDS, YMCA, or other organizations.

PADI Open Water Divers requires that students pass the Knowledge Development as well as Open Water Dives sections. Before diving, the student must sign a medical release from their physician. A photo must be submitted by the student diver to accompany his/her certification card. Digital images can be accepted.
